    Tell us a little about yourself or the person you are seeking support for. You don't need to have everything figured out before contacting us.

  2. Understand your needs

    Where appropriate, we arrange a care needs assessment to understand the person's needs, abilities, preferences, routines, home environment and relevant risks.

  3. Develop your care plan

    Together, we identify the support required and develop an individual care plan.

  4. Introduce your care team

    We identify suitable care professionals based on the person's assessed care requirements and other relevant considerations.

  5. Begin care

    Your agreed care service begins with clear communication between the individual, family where appropriate, care team and BAILE.

  6. Review & adapt

    Care needs can change. We maintain communication and review care so that changes can be considered when necessary.
